Your Concerns

How painful is the procedure?

Pain is not at all a concern. Only initial few pricks for a few moments you will experience very minimal pain, later no pain at all. For that also we have several pain minimising techniques to nullify it.

Any side effects like swelling or infections?

Even though swelling is well documented fact in hair transplantation surgery, we have devised techniques to nullify the incidence of swelling in our patients. At our centre we give a head band to patients to prevent the seepage of tissue fluids into the surrounding area there by lessening the chances of oedema/swelling due to the effect of gravity. It is a harmless condition and is not considered a dangerous complication as it resolves on its own within 2-3 days and no special medication is required to treat it.

Infection rate is very very negligible in our patients as we adapt stringent sterilization techniques are to be followed during pre operative and intra operative stages so as to minimize/avoid any chance of infection. Prophylactic antibiotics are given for a week following surgery to avoid infection.

Precautionary measures and post operative care is explained to each and very individual to minimize the risk of infection. Patients are advised to stay indoors for 24hrs following surgery in order to avoid exposure to the polluted environment as it gives scope to infection.

Any other complications?

As HT surgery involves just the superficial layers of the scalp there is hardly any chance of complication provided the surgery is performed well keeping all the possibilities into consideration. The surgery involves just 2-3 mm of very superficial layers of the scalp; hence damage to the underlying important structures is negligible. However one has to be well versed with the anatomical structures running in between the scalp layers so as to prevent the damage to any major blood vessel or a nerve in the vicinity which can give rise to uncontrolled bleeding or excruciating pain following the surgery.

Keloid could be a complication in patients with keloidal tendency if the surgeon has failed to take history about the healing of any previous injuries. Patients may experience some sort of discomfort at the suture site for few days following surgery which resolves after sometime.

Will there be considerable bleeding / loss of blood during the procedure?

Since scalp is highly vascular, the chances of bleeding are high. However, careful dissection and practicing newer techniques of hemostasis can prevent excessive blood loss and helps in achieving clean operating field during the procedure. We at Dr. Madhu's practice all the advanced techniques of hemostasis to ascertain minimum blood loss during the procedure. Hence one need not fear of excessive blood loss during the surgery. The preliminary tests i.e. bleeding time and clotting time helps us to delineate individuals with bleeding tendency and helps us to take necessary precautions to carry out the procedure safely or to discourage individuals with bleeding disorders from going for surgery.

How safe is the procedure?

e) HT is a cosmetic procedure done to improve the density of the existing hair or to give the necessary coverage over the bald area. Since it is not a life saving procedure one need not worry about the safety of the surgery. It's a safe procedure done as a day care surgery varying between 6-8 hrs done under local anesthesia.

Through out the procedure, the patient is conscious and awake, and can watch TV or listen music if he/she desires during the procedure. The expertise and skill of the surgery also helps in making the procedure as painless as possible. The drugs used for local anesthesia are also given in minimal doses so as to prevent untoward effects related to usage of higher doses. Meticulous sterilization and hemostasis are practiced. Continuous monitoring of vitals of the patient and frequent checking about pain or discomfort for the patient helps us to carry out the procedure smoothly. Our doctors are well trained in ACLS & BLS to attend to any untoward incidences or emergencies in the theatre if required.

What is newer technology? How is it beneficial?

Newer technology is URFUG. It involves removal of a strip of hair bearing scalp area (donor area) from back and sides of the head. Under Stereo and Mantis microscopes the strip is dissected carefully into follicular units which are then transplanted onto the bald area. This method is considered to be the gold standard in HT as this method helps in creating undetectable hair transplants. It is also cost-effective compared to other methods. This procedure enables the surgeon to perform Mega & Super Mega sessions in single sitting obviating the need of more no. of sessions.

There is no wastage of follicles and the survival rate of the grafts is maximum by this method. It helps in maximum coverage of bald area in single sitting which is not possible by other methods.

What is the difference between the older one? How does it differ from the present one?

The older version of HT involves plug type transplants giving artificial/detectable hair transplants. The newer methods of HT give a more natural looking transplant which mimics the original hair both in orientation and density, as a result even your friend or hair-dresser cannot detect that you have undergone hair transplant.

When can we expect the transplanted hair to re-grow?

Hair undergoes cyclic changes through out its lifetime involving growing phase, resting phase and falling phase. These cyclic changes are not synchronized. Each hair follicle will be under a different phase of cycle at a given time. Due to which, some of the transplanted hair may be in growing phase while some of them may be in resting (dormant) and few of them in falling phase. As a result, one should not expect all the transplanted hair to grow immediately after the procedure. It takes anywhere between 6-8 months to appreciate visible growth. One can also expect shedding of the transplanted hair which is in the falling phase within 4-6 weeks, which is a normal phenomenon.

In very few percentages of cases you can see hair growth as early as four months. A few percentage of patients may get satisfied by 6 months with the growth and in some cases it may take around 9-10 months. In general in most of the patients you can expect some appreciable growth on an average time frame of 8 months, but to attain full maturity it may take 12-18 months.

How many sessions are required to attain full density?

It depends on the extent of the bald area and the quality of the donor hair. The quality of the donor hair depends on the following factors:

a. Density
b. Thickness of the hair shaft.
c. Wavy nature of the hair.

If the quality of the donor area is excellent and the recipient area is less, then one can achieve maximum coverage in a single session, however if the bald area is more and donor supply is limited then one may require even two or three sessions to fill the entire area of baldness with moderate density.

Does the transplanted hair grows permanently?

Yes, the growth of the transplanted hair is almost permanent and life-long. The transplanted hair should behave exactly like donor hair as is taken from the donor area which is generally bald resistant & is not under the control of hormones which are responsible for theory of balding of hair.

Any special oils to maintain the growth?

Oils act as "Conditioners" and not growth promoting concoctions, hence no special oils are required to maintain growth as the growth is natural and permanent.

Is the post-operative dressing cumbersome?

Post operative dressing is very minimal (hair band similar to a tennis head band) and a surgical cap will be provided which needs to be worn for 3 days.

However, if you are opting without disturbing your existing hair it incurs extra charges, as this procedure demands greater precision, skill and time also.

Do I have to use any medicines after the surgery?

For transplanted hairs, no medication is required. Offcourse routine analgesics and antibiotics will be provided for 7 days (complimentary). However, it is better to take medication after the surgery as it will help in retaining existing hair which continues to be under the influence of hormones. In order to retain those hairs and to prevent further progression of baldness, it is advisable to take medication for longer duration, under doctor supervision. However, the choice is left to the individuals.

Any antibiotics need to be taken after the surgery?

Prophylactic antibiotics are given for 7 days following surgery to prevent any infections.

Any pain killers need to be administered after the surgery?

Routine analgesics are given for 4 days along with antibiotics to administer as and when required.

Will my surgery be performed in a sterile ambience?

Of course, the entire procedure is carried out under highly sterile environment.

How to stop existing hair fall?

Is there any age limit to undergo Hair transplantation?

As Hair transplantation is a cosmetic surgery to improve the aesthetic appeal, there is no age limit provided, the person is medically fit for surgery.

Which consultation is better? Personal or Online?

Personal consultation is always better as it helps us to examine the scalp and hair clinically to exclude any other cause of hair-loss. It also helps to examine the quality of your donor hair to assess your suitability for the procedure and explain the best possible outcome.

It helps us to know your expectations with regards to the hair line design and the area of coverage. And above all it gives us a scope to build rapport with you. Hence we strongly recommend personal consultation rather than or even after an online consultation.

What will be frequency of my follow-up visits to the clinic after the procedure?

Do you provide any accommodation for out-station patients?

We can guide you regarding accomodation.

What is the survival rate of grafts implanted?

As we are follwoing the most advanced methods like Ultra Refined Follicular Unit Grafting Method and so many sophisticated methods to increase the survival rate. All our patients are getting very good results. With these methods one can expect very good survival rate even sometimes 100%, but the survival rate varies inbetween 90% to 100%, on an average one can take 95% survival rate in routine cases .

What is the success rate of the surgery?

The success rate of the surgery depends upon the following factors:

a. Quality of the donor hair.
b. The extent of the recipient area
c. Expertise and skill of the surgeon.
d. Duration of the procedure.
e. Intrinsic factors like

1. Prolonged solar damage of the scalp
2. X-factor (chromosomal)
3. Smoking habit of the patient
4. Usage of drugs like Anti-psychotic drugs, Anti metabolites etc

If all the above criteria are met successfully, then one can achieve a success rate of > 95%. We at Dr. Madhus enjoy the success rate of 98%.

What is the major factor that determines the output?

The major factors that determine the outcome of procedure are

1) No. of grafts extracted from donor area more the no better the results.
2) Quality of donor hair

a) Diameter of hair
b) Curly nature of hair
c) Hair and skin contrast ratio

3) Implantation of grafts according to the natural orientation
4) Density of implantation.
5) The extent of the recipient area
6) . Expertise and skill of the surgeon.
7) Duration of the procedure.
8) Intrinsic factors like

a. Prolonged solar damage of the scalp
b. X-factor (chromosomal)
c. Smoking habit of the patient
d. Usage of drugs like Anti-psychotic drugs, Anti metabolites etc

Note: Lesser the recipient area and denser the donor area, the better the outcome of the surgery.

How to compare the quality of Hair transplantation?

The quality of the hair transplantation is assessed by the following factors:

a. Hairline designing
b. Extent of coverage
c. Pattern of grafts placement.
d. Healthy skin in between the transplanted grafts
e. Undetectable and natural looking hair transplants after growth.
f. Minimal or invisible scar of the donor area after healing.

Why very few doctors are going into this field for such a demanding procedure?

1. Immense concentration and patience are required to perform these lengthy procedures.
2. Identifying, training and maintaining a huge team of surgical assistants is no mean task.

3. Should not remove the head band for 72 hrs.

Do I develop Thicker scar at the donor site?

As we are adapting the latest method tricophytic closure method and more refined techniques most of our patients are getting almost negligible/ Undetectable scars. Very rarely depending upon the individual healing characters and nature of skin (e.g.keloid) some patients may get detectable scars than expected.

Will it affect brain and cause mental abnormalities?

The surgery involves only the superficial layers of the scalp and not the underlying bone and structures. Further more, it is very difficult to damage the skull bone and cause any injury to the internal structures like vessels & nerves. So, there is no scope for mental disturbance or abnormalities or any such related problem following the procedure

What is scar-less surgery?

Some people are claiming FUE technique as scar less surgery, it is better to call as scalpel less surgery rather than scar less surgery. Even though FUE doesnt produce a linear scar it leaves dot like scars randomly which gives mottled appearance in the donor area.

What is trichophytic closure?

Dr. Madhu is the first person to introduce this technique in India (in 2006) when it was still in the evolution stage. Trichophytic closure technique is a modification of surgical closure technique to minimize the scar and at the same time to promote hair growth through the scar. Generally, all the scars heal with fibrosis leaving a bald area. But by adapting this technique, one can expect re-growth even through the bald scar; thereby making the scar almost invisible and undetectable as it is masked by the growing hair.

Following this closure technique, 90% of hair transplant patients are getting minimal or negligible scar over the donor area. Only Less than 10% of patients may get detectable scars than expected despite the usage of this technique due to connective tissue variations and keloid tendencies which differ from person to person and also the tension at the wound site making the scar obvious.

What is the difference between hair follicle and follicular unit?

A hair follicle consists of a single hair where as a follicular unit may contain one hair, two or three or sometimes even four follicles or hair.

Some people charge per hair, but here we charge per follicular unit and here one should not get confused between hair and follicular unit.

3000 follicular units may amount to 5000-6000 individual hairs.

What is multi-follicular unit?

A follicular unit (FU) which contains more than 2 hair follicles is called as a Multi follicular unit.

I am confused between FUT & FUE which one should I opt for?

FUT (Strip) is like a straight and simple and most effective method for covering larger bald areas. With this even in a single session we can implant more number of grafts, sometimes amounting to even 5000 grafts. more

Any special shampoos should be used after the procedure?

Shampoos just act as cleansers. They do not have any growth promoting agents in their composition. Hence, no special shampoos are required after the procedure. However antiseptic shampoos are advised for 2 days before the surgery to prevent any infection.

What is hair cloning? Can I think about that?

Lot of research and speculation Is going on in this field, but presently there are many limitations for cloning to become a reality. Even if it turns reality, it will become a very costly affair. Otherwise, it can be compared to a human expedition to mars. It may take several years to give fruitful results for a common man.
